My patient safety experience centers around being courageous enough in a hierarchy (nurses to doctor) to “stop the line” in a situation that was procedural and scary. Without too many personal details, the patient was not harmed, although required a second procedure, which should not have happened. The physician has since been enlightened, the unlicensed team are not afraid to speak up- (modeling) and we are on the road to better exceptional patient care.

My son had surgery at three years old and our orthopedic surgeon (who is amazing) gave us his cell phone number and told us to call him if we needed anything our vacation to Mexico. My son had just had the surgery and was still in a cast and he (his surgeon) was concerned enough about his care and safety that he even wanted to us to call while we were in Mexico.

My daughter recently had a baclofen pump placed. Several days after discharge she had a fever and rash. We took her back in and the ordered an ultrasound. After waiting for over four hours, we finally went to have the ultra sound. As the tech entered my child’s information into the computer images of a recent scan (within two hours) appeared and the tech said that it looked like she already had an ultrasound. After a thirty minute conversation to convince him we didn’t it was uncovered that another child’s images were accidentally uploaded to my daughter’s record.
We had conversations that day and within the next few weeks to uncover together how the error occurred and how we might be able to prevent it in the future with other patients.
